Dr. Horowitz is an Assistant Professor of Internal Medicine (General Medicine). She received her Medical degree from Harvard Medical School and obtained a Master’s in Clinical Research and Epidemiology from Yale University. She completed her residency in Internal Medicine at Mount Sinai Medical Center where she was Chief Resident. She was a Robert Wood Johnson Clinical Scholar at Yale University and an attending physician at both the Veteran’s Administration Hospital and Yale New Haven Hospital. Dr. Horowitz’s plans to build a career at the intersection of health services research and bedside care – to design rigorous, evaluable interventions that promote high quality care and improve patient safety on the ground.
Dr. Horowitz’s project, Assessing the quality of the discharge process and it's impact on clinical outcome, assesses the quality of communication between inpatient providers, outpatient providers and patients and plans to use the data generated by this pilot study to develop preliminary hypotheses about the effect of discharge communication quality on adverse outcomes.
